What forms do I need to fill out?
Prior to seeing the doctor, you’ll be asked to fill out a patient history, privacy and financial form. For your convenience, please look for the link sent via email or text message.
What cards should I bring?
Please bring your drivers license or state ID, and (if you have one) your dental insurance card.
Where can I park?
There is street parking and parking behind the building.
Can I transfer records from another dentist?
If you are a new patient, please call your old dentist to have your records emailed to our office: canfield@premierdentalohio.com
