What to do before your appointment.

To make your experience a happy one, here are some answers to FAQs before you meet your dental team.

• What forms do I need to fill out?

  Prior to seeing the doctor, you’ll be asked to fill out a patient history, privacy and financial form. For your convenience, please look for the link sent via email or text message.
  

• What cards should I bring?

  Please bring your drivers license or state ID, and (if you have one) your dental insurance card.
  

• Where can I park?

  Our facility features plenty of parking in front of the building, including handicap accessible spots with easy access to the entrance.

• Can I transfer records from another dentist?

  If you are a new patient, please call your old dentist to have your records emailed to our office: sunbury@premierdentalohio.com

  If they have questions, please ask them to call (740) 913-0333.

• What insurance do you work with?

  We accept all people and all forms of payment. We will help you get the most of your insurance; click here for a list of insurance we work with.
  

• Should I brush my teeth before my appointment?

  Yes. :)
