To make your experience a happy one, here are some answers to FAQs before you meet your dental team.

  • What forms do I need to fill out?

    Prior to seeing the doctor, you’ll be asked to fill out a patient history, privacy and financial form. For your convenience, please look for the link sent via email or text message.

  • What cards should I bring?

    Please bring your drivers license or state ID, and (if you have one) your dental insurance card.

  • Where can I park?

    Our office is located on Highland Avenue and offers plenty of parking on either side of the building, including handicap accessible spots with easy access to the entrance.

  • Can I transfer records from another dentist?

    If you are a new patient, please call us at (740) 335-2271 to begin the process.

  • What insurance do you accept?

    We accept all insurance. We will help you get the most of your insurance.

  • Should I brush my teeth before my appointment?

    Yes. :)
